What is gastric pouch resizing and why is it performed?
Weight loss surgery (also called bariatric) has a growing role in management of obesity which is now killing more people in the world than underweight. Gastric bypass is the most frequently performed weight loss surgery in the world. Although it has a significant evidence based positive effect on weight loss and obesity related comorbidities, a long-term follow-up revealed that some patients regain weight a few years after the surgery. Corrective surgical techniques were developed to solve this problem.
- Gastric pouch is a small sack which is formed from the upper part of your stomach during the gastric bypass surgery. This newly created stomach is only 15 to 30 ml in volume so you feel satiated with smaller amounts of food. Consequently, your caloric intake decreases and you lose weight.
- However, in some cases the gastric pouch may enlarge and your food consumption starts increasing as well as you weight. The approximate rate of weight regain after gastric bypass is 20,4 % in morbidly obese (BMI 40-40.9 kg/m2) and 34,9 % in super obese (BMI 50-59.9 kg/m2) patients. It happens due to various determinants, including eating behaviour, lifestyle, hormonal and surgical factors.
- Your doctor may suggest upper gastrointestinal endoscopy or other visualising methods in order to confirm pouch dilation. Dilated gastric pouch can be successfully reduced during a surgery called gastric pouch resizing.

How is it performed?
Gastric pouch resizing is a minimally invasive (laparoscopic) procedure. This means that it does not require large incisions (laparotomy). This approach includes several small incisions and several hollow tubes called laparoscopic trocars. The surgeon inserts all instruments through the trocars and visualizes your abdomen with the help of a thin camera. This surgery is performed under general anaesthesia, so you will not feel, hear or remember anything what will happen during the whole procedure.
What to expect after gastric pouch resizing?
The average hospital stay varies between 2 to 5 days, but usually patients are discharged on the second day after the surgery. This time is enough to remove drains, change wound dressing and rule out possible complications. Your doctor will prescribe you some medications including proton pump inhibitors (drugs reducing the production of acid in your stomach), multivitamins, calcium, iron and B group vitamins.
