Gastric sleeve surgery is among the most popular bariatric surgeries. Usually, it is the last choice for those struggling to lose excess body weight. After 1-1.5 years post-surgery, weight loss naturally slows down and then as many as 30% of patients give up their healthy habits. It is known that for the first 5 years after surgery patients are more prone to regain weight.
It works wonders for some patients who lose as much as 65% of their excess weight, however, others cannot lose enough weight even after undergoing the surgery. There are several key differences between these two groups of people
Successful patients exercise regularly, avoid “comfort food”, follow a balanced diet, and eat in small portions. They feel responsibility for their own weight loss and have completely changed their mindset as well as everyday habits.
On the other hand, patients who fail to lose weight stick to previous lifestyle choices even after weight loss surgery. They continue leading a sedentary lifestyle and eating unhealthy foods in large quantities which leads to complications and weight gain.

Good revision surgery candidates are those:
Patients who fail to lose enough weight after undergoing gastric sleeve surgery should firstly consult a doctor. The doctor will evaluate the patient‘s health state and determine what is the reason for gastric sleeve failure (physiological, psychological, or lifestyle related).
If the gastric sleeve did not work because of poor lifestyle habits, a patient can choose to be re-educated by a dietitian or a psychologist. Learning how to choose healthy food options, eating small portions, and exercise regularly may be all that is needed for the gastric sleeve to work.
Unfortunately, surgery is almost always indicated if the reason for gastric sleeve failure is physiological. Side effects, such as esophageal or gastric dilation, reflux, or strictures should be treated surgically.
Someone planning a revision surgery may consider several available options which include revisional gastric sleeve (e.g. fixing a dilated gastric sleeve), converting to a gastric bypass, mini gastric bypass, or duodenal switch. The latter one, however, is usually not performed in standard cases because of a high complication risk.
Gastric bypass and mini gastric bypass would be the most recommended surgeries for a gastric sleeve revision patient. Gastric bypass is both restrictive and malabsorptive, making it a much more effective two-way weight loss method (note that gastric sleeve is just a restrictive procedure). The main advantage of this surgery is that it is permanent, weight loss is experienced immediately, and there are no post-surgery adjustments needed for this weight loss method to provide the desired results, however, lifestyle and nutritional habit changes are still essential for the surgery to work long-term:
Additional surgery is associated with a higher complication risk, and thus, we suggest choosing a highly experienced bariatric surgeon. All patients should follow a presurgery weight loss diet to reduce intra-operative complication risk. Some of the possible risks are:
In order to ensure long-term success of the surgery, patients should adopt life-long behavioral changes. It is necessary to eat very small meals, exercise regularly, and take vitamin supplements and avoid overeating, consuming foods high in sugar and fat, or carbonated drinks. Other unhealthy habits, such as tobacco or alcohol consumption, also have a strong influence on the weight loss results, and thus, should be strictly avoided.
Preparation for the surgery begins with a consultation with your bariatric surgeon who determines the best treatment option for you. There is a number of medical tests that will need to be performed, such as a complete blood count, urine analysis, electrocardiogram (ECG), and gastroscopy. In order to determine any underlying problems (e.g. liver damage, kidney failure, or high blood cholesterol levels), you might be asked to do lung or kidney function tests.
Smoking and alcohol consumption should be ceased one month before surgery. You should refrain from alcohol for at least the first postoperative year.
Also, be aware of blood-thinning medications (e.g. aspirin, warfarin, and ibuprofen) and substances (e.g. fish oil or garlic), as they may need to be discontinued before surgery. Always be honest about the medication your taking with your medical team and inform them to seek advice on how to take them when preparing for surgery. Get into the habit of taking multivitamins daily and exercising regularly (e.g. walking fast-paced) for at least 90 minutes a day.
A low-calorie diet is advised before surgery to achieve presurgery weight loss and prepare you for the postoperative diet.
On the day of the surgery, you should wash yourself with anti-bacterial soap. You will be required to fast 6 hours before surgery and do not drink any water 2 hours before surgery.
Gastric bypass, as well as gastric sleeve diets, apply the same post-bariatric surgery menu principles. After surgery diet usually follows a staged approach to help you gradually adapt to eating solid foods again.
On the first week post-surgery, you will only be able to consume clear liquids (water and juices). At the clinic you will also be given baby puree and low-fat natural yoghurt, which you can consume the first week. Depending on how well you can handle clear liquids you may have other liquids (Jell-O, diluted broth, skim milk). Drinks high in sugar, carbonated or caffeinated drinks should be strictly avoided as they can cause dumping syndrome. In order to prevent anemia and vitamin deficiencies, you must take vitamin and mineral supplements daily.
On the second week post-surgery, you should begin fully eating pureed foods. Examples may include pureed soups, very soft non-fibrous fruit and vegetables that may be blended with water, skim milk, or broth. It is highly important to eat often (5-6 times a day) and in very small portions (4-6 tablespoons). You should set aside at least 30 minutes to chew and swallow slowly.
After two weeks of pureed food, you may slowly proceed to soft foods, such as flaked fish, scrambled eggs, cottage cheese, rice, or soft poultry. You should eat 3-5 meals a day, no more than 100-150 ml or grams per serving.
After two months on the bariatric diet, you can return to eating solid foods. Note that you should still eat often (3-5 times a day) and consume small portions of food. A general rule to follow is to stop eating before feeling completely full. Bear in mind that certain foods (e.g. bread, fizzy drinks, fibrous fruits and vegetables, fried or spicy food, popcorn) may cause nausea and vomiting. It is advised to completely eliminate easy carbohydrates and limit fruit intake to a minimum.
Gastric sleeve revision surgery is usually performed laparoscopically, and thus, there will be around five small incisions in your stomach area which allows for shorter recovery time. It is normal to experience swelling and bruising, however, if you experience bright red skin, excessive fluid or blood leakage from postoperative wounds, contact your doctor immediately.
Most patients spend approximately 2-3 days at the clinic and can return to regular activities within 3 to 6 weeks. After one day post-surgery, you should begin walking and have a shower after 5 days post-op. It is recommended to take at least 2 to 4 weeks off work to rest and heal at home.
Pain is managed with over-the-counter drugs, such as paracetamol. Note that anti-inflammatory drugs such as ibuprofen are not advised after such surgery, as they may irritate the stomach. Keep your wound dressings clean, use waterproof plasters when showering and exercise sterile wound care.
